ScienceDaily (Sep. 23, 2011) ? Until recently, options for patients with bone metastases from advanced prostate cancer have been very limited. But now the first Phase III study of an alpha-pharmaceutical in these patients has shown that it can prolong survival significantly, according to research reported September 23 at the 2011 European Multidisciplinary Cancer Congress.
Dr. Chris Parker, Consultant Clinical Oncologist at the Royal Marsden Hospital, London, UK, told the congress that the ALSYMPCA international Phase III study of the drug Radium-223 Chloride (Alpharadin TM) in 922 prostate cancer patients who were resistant to hormone treatment and had bone metastases, had been stopped early once an interim analysis by the Independent Data Monitoring Committee (IDMC) in June 2011 had revealed that patients receiving the best standard treatment plus radium-223 were living longer than those who were receiving the same standard treatment plus placebo. The hazard ratio was 0.695, (p = 0.00185), meaning that patients taking radium-223 had a 30% lower rate of death compared to patients taking placebo. Median overall survival for patients taking radium-223 was found to be 14 months, compared with 11.2 months in the placebo group. "It would have been unethical not to offer the active treatment to those taking placebo," Dr. Parker said.
Alpha-pharmaceuticals work by delivering minute, highly charged and targeted doses of damaging radiation to a secondary tumour (metastasis) in the bone. Radium is similar to calcium in that it sticks to bone, and particularly to where new bone is being formed, so it is a highly effective way of delivering radiation to a target. "It takes only a single alpha particle to kill a cell," Dr. Parker explained, "and collateral damage is minimised because the particles have such a tiny range - a few millionths of a metre (micrometres). So we can be sure that the damage is being done where it should be, to the metastasis, and very limited elsewhere."
The researchers chose to study the drug in patients with prostate cancer because of its high tendency to metastasise to bone. Around 90% of all men with prostate cancer will develop bone metastases in the advanced stage, and in many cases there are not detectable metastases elsewhere in the body. The safety profile of radium-223 was found to be highly favourable, the researchers say.
"Compared to chemotherapy, which affects all the tissues of the body, radium-223 is highly targeted to the bone metastases, and it has a completely different safety profile," said Dr Parker.
Side effects with radium-223 are minor. It can cause nausea, and occasional loose bowel movements, and there is a very small effect on the bone marrow. "Although it has never been rigorously compared with chemotherapy, from observing patients in the clinic it is clear that patients tolerate it much better than they do chemotherapy," Dr. Parker said.
The researchers now intend to submit the data for regulatory approval. "I would hope that the authorities will approve radium-223 as a treatment for bone metastases in advanced prostate cancer soon," said Dr. Parker. "This is a common cancer - the second commonest cancer killer in men in the UK - and so it's a big disease burden. We urgently need effective treatment for it.
"I have no doubt that there will be further trials looking at a combination of radium-223 with other drugs that are currently used in prostate cancer, and that there will also be studies using radium earlier in the disease. In particular, our research was restricted to those men who were not going to receive chemotherapy for prostate cancer. It would be interesting to use radium-223 chloride before chemotherapy, since it might be even more effective in that setting.
"Additionally, the drug could be used in many other types of cancers which metastasise to bone, regardless of the primary site. We believe that our trial may have paved the way for improvements in survival for very many cancer patients," he concluded.

ScienceDaily (Sep. 23, 2011) ? Until recently, options for patients with bone metastases from advanced prostate cancer have been very limited. But now the first Phase III study of an alpha-pharmaceutical in these patients has shown that it can prolong survival significantly, according to research reported September 23 at the 2011 European Multidisciplinary Cancer Congress.
Dr. Chris Parker, Consultant Clinical Oncologist at the Royal Marsden Hospital, London, UK, told the congress that the ALSYMPCA international Phase III study of the drug Radium-223 Chloride (Alpharadin TM) in 922 prostate cancer patients who were resistant to hormone treatment and had bone metastases, had been stopped early once an interim analysis by the Independent Data Monitoring Committee (IDMC) in June 2011 had revealed that patients receiving the best standard treatment plus radium-223 were living longer than those who were receiving the same standard treatment plus placebo. The hazard ratio was 0.695, (p = 0.00185), meaning that patients taking radium-223 had a 30% lower rate of death compared to patients taking placebo. Median overall survival for patients taking radium-223 was found to be 14 months, compared with 11.2 months in the placebo group. "It would have been unethical not to offer the active treatment to those taking placebo," Dr. Parker said.
Alpha-pharmaceuticals work by delivering minute, highly charged and targeted doses of damaging radiation to a secondary tumour (metastasis) in the bone. Radium is similar to calcium in that it sticks to bone, and particularly to where new bone is being formed, so it is a highly effective way of delivering radiation to a target. "It takes only a single alpha particle to kill a cell," Dr. Parker explained, "and collateral damage is minimised because the particles have such a tiny range - a few millionths of a metre (micrometres). So we can be sure that the damage is being done where it should be, to the metastasis, and very limited elsewhere."
The researchers chose to study the drug in patients with prostate cancer because of its high tendency to metastasise to bone. Around 90% of all men with prostate cancer will develop bone metastases in the advanced stage, and in many cases there are not detectable metastases elsewhere in the body. The safety profile of radium-223 was found to be highly favourable, the researchers say.
"Compared to chemotherapy, which affects all the tissues of the body, radium-223 is highly targeted to the bone metastases, and it has a completely different safety profile," said Dr Parker.
Side effects with radium-223 are minor. It can cause nausea, and occasional loose bowel movements, and there is a very small effect on the bone marrow. "Although it has never been rigorously compared with chemotherapy, from observing patients in the clinic it is clear that patients tolerate it much better than they do chemotherapy," Dr. Parker said.
The researchers now intend to submit the data for regulatory approval. "I would hope that the authorities will approve radium-223 as a treatment for bone metastases in advanced prostate cancer soon," said Dr. Parker. "This is a common cancer - the second commonest cancer killer in men in the UK - and so it's a big disease burden. We urgently need effective treatment for it.
"I have no doubt that there will be further trials looking at a combination of radium-223 with other drugs that are currently used in prostate cancer, and that there will also be studies using radium earlier in the disease. In particular, our research was restricted to those men who were not going to receive chemotherapy for prostate cancer. It would be interesting to use radium-223 chloride before chemotherapy, since it might be even more effective in that setting.
"Additionally, the drug could be used in many other types of cancers which metastasise to bone, regardless of the primary site. We believe that our trial may have paved the way for improvements in survival for very many cancer patients," he concluded.

Q: Because I believe that pitching wins in the playoffs, I think the World Series winner will come from the NL this season. What do you think? ? Sean Royson, Milwaukee
A: The only thing we know about the postseason right now is that the Phillies will go in as the favorites ? and as you mention, the reason will be their dominant rotation.
It really will be hard to pick against a team that will run out Roy Halladay, Cliff Lee, Cole Hamels and most likely, Roy Oswalt in a short series.
But we saw last October that the Phillies offense can be shut down, and upsets happen all the time. And with so much balance among the eventual eight playoff teams, the term 'upset' barely will be applicable next month.
As for who can knock off the Phillies ? and speaking further to your point ? I've got two words for you: Justin Verlander. Nobody is pitching better than the Tigers' ace, and as we've seen so many times in the past, it's often a great starting pitcher or two leading the way to a World Series title.
Verlander is having one of those seasons for the ages ? like Tim Lincecum last year, Bob Welch in 1989, Orel Hershiser in 1988, and many, many others through the years ? and the Tigers have enough pieces to go with him to win it all.
In fact, I'd argue they are a better team than their 2006 AL pennant winner that gave away the World Series with critical fielding/throwing errors by their pitchers.
From the National League, the Brewers will run out the second-deepest rotation in Zack Greinke, Yovani Gallardo, Shawn Marcum and Randy Wolf. But in the AL, don't overlook the Rangers, who will chose from among five 13-game winners in C.J. Wilson, Colby Lewis, Derek Holland, Alexi Ogando and Matt Harrison.
But at this point, we don't even know who the final couple of playoff teams will be, as the Red Sox and Braves may not be able to hang onto spots everybody thought were wrapped up a couple of weeks ago.
It's not a coincidence that both of those teams have starting pitching issues down the stretch. In fact, in terms of depth, you'd have to give the edge to the Rays' rotation over the Red Sox's, and to the Cardinals' rotation over the Braves', which is without Jair Jurrjens and Tommy Hanson.
Q: Charlie Manuel said if he had his choice, the Phillies would play the Yankees in the World Series. Is that wise of him to wish that? ? Andy Washington, Brooklyn
A: I don't know the context of that quote, but knowing Charlie, I'm thinking he was looking at it in terms of having an opportunity to avenge the 2009 World Series loss ? as opposed to thinking the Yankees are a potentially weaker opponent than the other AL playoff entrants.
Also, the Yankees in the World Series automatically makes it a bigger event, and the proximity of the two cities makes it an even more appealing match-up, in my opinion. So I see where Charlie is coming from in that respect.
But it's not hard to build a case for the Yankees being the toughest of the AL playoff teams ? led by their offense and bullpen. Their formula is simple: Get a lead after the sixth inning, then turn it over to David Robertson, Rafael Soriano and Mariano Rivera.
But I'll continue to believe ? as I have all season ? that the Yankees' rotation after CC Sabathia makes them much more vulnerable in October than in the regular season, when they have out-slugged the weaker teams.
Q: What is your take on the Phillies rotation in the playoffs? Should Vance Worley or Roy Oswalt be in the rotation? ? Mike Davis, Philadelphia
A: The raw 2011 numbers give the edge to Worley. The 'Vanimal' has been better than the veteran in terms of W/L record, ERA, hits per nine innings, strikeouts per nine innings and WHIP.
But now that Oswalt is over back problems that put him on the disabled list mid-season, you have to think he'll get the call as the Phillies' No. 4 starter in the playoffs.
Oswalt's career ? which includes a 5-1-3.39 postseason line ? speaks for itself. And since coming off the disabled list in August, he has gone 4-3-3.50 and 2-1-3.29 in his last four starts, including seven shutout innings against St. Louis on Saturday.
Worley has made a couple of relief appearances in his brief major-league career ? and even picked up two holds this season. He also has said he'll have no problem with the shift the bullpen during what is expected to be a deep Phillies' run this October.
After winning 11 consecutive decisions, Worley has lost his last two, against Milwaukee and St. Louis ? although he pitched well in both (12.2 IP, 4 earned runs). As for his role in the bullpen, it could turn out to be much more than just some middle-inning work, as depth has been an issue at times this season.
Michael Stutes and Antonio Bastardo have been worked hard, and have shown some signs of wearing down, while Brad Lidge remains at something less than 100 percent. So don't be surprised to see Worley in some crucial seventh- and eighth-inning situations.
And come to think of it, Oswalt also could be used some in relief, as he figures to make only one start per series.

Molecular pathways could play important role in the fight against obesity and diabetes
BOSTON September 20, 2011 Researchers at the Joslin Diabetes Center have identified for the first time two molecular pathways that are critical to activating a type of "good" fat found in the body, a discovery that could play an important role in the fight against obesity and diabetes.
The fat, called brown fat, actually burns energy rather than storing it, which the more common white fat does.
The study, published in the October issue of Endocrinology, sought to learn more about how to get brown fat cells to grow. It identified two molecular pathways that lead to a protein called necdin that blocks brown fat growth.
With this information, researchers can look for ways to modify the steps along the pathways, either to stimulate another protein, called CREB, which shuts down necdin, or block a different protein called FoxO1, located along the second pathway, which stimulates necdin. The study showed for the first time that the two proteins can bind directly to the necdin gene.
"This is a very important piece of the puzzle," said Aaron Cypess, MD, PhD an assistant investigator and staff physician at Joslin and lead author of the paper. "It provides new opportunities. The point is that we have got to learn how to grow these brown fat cells. There's a lot of missing information. We filled in some of the important missing pieces."
Based on previous research, including a 2005 paper by Yu-Hua Tseng, Ph.D., a Joslin investigator who also is senior author of today's paper, there was some evidence that the two pathways were important to the process of brown fat growth.
In today's study, the researchers conducted tests in vitro on different cell lines derived from brown fat taken from mice. "We used different drugs to stimulate or block the signaling pathways that we thought were important," Dr. Cypess said. "The result was that we defined the two pathways. We found what goes to what to cause something to happen to the cells."
One pathway to necdin starts with insulin cells and runs through proteins called Ras and ERK1/2 before getting to CREB. The second also starts with insulin and runs through proteins called P13-K and Akt before getting to FoxO1.
"Both pathways get you there," Dr. Cypess said.
"With this more detailed description of the pathways leading to (brown fat tissue), there can be more focused attempts to develop interventions using brown fat as a treatment for obesity and diabetes," the paper concludes.
One intervention could be to grow brown fat in a laboratory and transplant it into the bodies of people who need it. Another could be the development of drugs to stimulate brown fat growth.
This is the latest in a series of studies on brown fat led by Drs. Cypess and Tseng. In July, Dr. Cypess and his team showed that brown fat can be seen on imaging studies in nearly half of all children and is most active in those who are thin. The amount of the fat also increases in children up until puberty, when it begins to decline, according to that study, published in the Journal of Pediatrics.
In 2009, Dr. Cypess and his team demonstrated in the New England Journal of Medicine for the first time that brown fat is metabolically active in adult humans. Previously, it had been thought that brown fat was present only in babies and children. The 2009 study showed it was found to be active under normal living conditions in 5.4 percent of all adults, with higher rates in women.
A 2008 Joslin study published in Nature by Dr. Tseng and colleagues discovered that a protein called BMP7 could induce brown fat formation. Another more recent 2011 study from Tseng's group identified precursor cells in mice that can be triggered by BMP7 and other inducers to transform into brown fat.
Concerning the small percentage of people in whom brown fat was detected in the 2009 imaging study, Tseng said it is possible that a much higher percentage of people have brown fat -- possibly everybody -- but that it simply was not able to be detected in the study because the scanners were not sensitive enough or because it might not have been activated in most people.
Either way, she said she considers the study of brown fat to be a "very important" issue because it offers a potential treatment for the obesity epidemic.
"Brown fat burns energy," she said. "It is a special tissue. These studies have opened up a new avenue for the treatment of obesity and its related disorders. This study will help us deepen our understanding of brown fat formation and could in the future combined with other information that we have learned be used to develop drugs or other interventions for obesity. But we still need to know more."

SINGAPORE (AP) ? Oil prices fell to below $87 a barrel Monday in Asia as investor concerns about Europe's debt crisis fueled a stronger U.S. dollar.
Benchmark oil for October delivery was down $1.12 at $86.84 at late afternoon Singapore time in electronic trading on the New York Mercantile Exchange. Crude dropped $1.44 to settle at $87.96 on Friday.
In London, Brent crude for November delivery was down 22 cents at $112.00 on the ICE Futures exchange.
The euro has weakened this month amid growing fears that a possible debt default in Greece could trigger similar moves in Portugal, Ireland, Italy and Spain as well as a banking crisis. A stronger dollar makes commodities such as oil more expensive for investors with other currencies.
The euro fell to $1.3698 on Monday from $1.3794 on late Friday.
"We still view euro zone debt issues as challenging to the extent that a long term solution will remain elusive," energy consultant Ritterbusch and Associates said in a report. "The euro will be proceeding through a renewed weakening phase that will be placing additional pressures on the oil market."
Investors this week will also be closely watching the outcome of a U.S. Federal Reserve meeting and corporate quarterly earnings results.
In other Nymex trading for November contracts, heating oil fell 1.0 cent to $3.01 per gallon and gasoline futures slid 1.3 cents to $2.75 per gallon. Natural gas for October delivery fell 0.7 cent to $3.80 per 1,000 cubic feet.
This should look familiar to Philadelphia Eagles fans. In the third quarter of Sunday night's game with the Eagles clinging to a 24-21 lead, Falcons defensive back Dunta Robinson made an extremely dirty play and delivered a brutal hit on an Eagles receiver.
Michael Vick threw a laser to Jeremy Maclin in the middle of the field, but as he reeled in the catch, Robinson led with his helmet and nailed him in the head.
"Unbelievable! Unbelievable that he has done it again," NBC broadcaster Chris Collinsworth said after the play as Maclin laid on the ground. Collinsworth was referring to last season's game between the two teams, when Robinson made a similar hit on DeSean Jackson.
Fortunately, Maclin appeared to be okay and returned later in the game. Despite the big hit, Maclin finished with 13 receptions for 171 yards and two touchdowns. But the Falcons went on to win 35-31. (WATCH HIGHLIGHTS HERE)
"He's two for two now," Maclin said of the hit. "Fortunately, it wasn't as bad as Jack's was last year. I guess we'll see what Roger Goodell has in store for him."
Last year, Jackson suffered a severe concussion and some memory loss. Robinson was fined $50,000.
After the game, Robinson said he thought the hit was legal and that "there will not be any further repercussions"
Others disagree.
ESPN's Chris Mortensen suggested that Robinson could even be suspended. Gregg Rosenthal of ProFootballTalk also suggested a possible suspension since Robinson is a repeat offender.
